Patient Registration

Please bring your medical card to reception. If you cannot find your medical card, please speak to the receptionist.

All newly registered patients will be asked to complete a questionnaire and invited to attend for a “new patient” check up/assessment with the nurse practitioner initially. This enables us to meet you and to assess your medical needs.

Please note that you can choose to attend any of the doctors in the partnership, although we would encourage you to consult one partner for chronic problems as this helps to ensure continuity of care.

Please ensure that the new patient questionnaire is fully completed, where possible, and given to the receptionist in good time, prior to your first appointment. This ensures that the nurse practitioner or doctor has some information about you.
